Proposal Format
Proposers shall submit one copy of their technical proposal electronically in the Housing Agency Marketplace or in a sealed envelope to MPHA’s Procurement Department. Proposed fees must be submitted online in the Housing Agency Marketplace.

MPHA intends to retain a Contractor using a best value basis, meaning MPHA will consider factors other than just cost in making an award (see Section 8 in the RFP document for the evaluation factors). Therefore, to allow for easy comparison of proposals during evaluation, proposals should contain the following information and documentation and be arranged in consecutive order.
Tab 1: HUD Form 5369-C (Attachment A)
This form must be fully completed and submitted as part of the proposal submittal.
Tab 2: Profile of Firm Form (Attachment B)
This form must be fully completed and submitted as part of the proposal submittal.
Tab 3: Background Information and Experience
Provide background information on your organization, including:
a. Name, Address, and Tax Identification Number of the firm or consultant.
b. The number of years the firm or consultant has provided the consultation services listed in the scope of work.
c. The total number of employees and organizational structure.
d. Any industry reports, recognitions, certifications, and/or articles regarding the organization's work in retirement plan consulting.
e. A completed investment advisory Form ADV Part I and Part II (including Schedule I). Confirm that your organization is an independent advisor as described in the scope of work.
f. Provide information on the individuals who will work on MPHA's account, including their qualifications, education, skills, and experience.
Tab 4: Current Clients
Provide information on your organization's current clients, including:
a. The types and sizes of retirement plans that you provide consulting services for and if you have experience with defined contribution plans, deferred compensation plans, and/or 401(a) Plans.
b. A brief description of the services provided and the plan size and type for three current clients and a contact name and phone number for each of these client references.
c. Provide two specific examples of ways your firm has made a positive impact on retirement plans you serviced over the last two years. Examples may include enhanced services, lowered fees, or others.
Tab 5: Proposed Services
Provide information on the following:
a. Identify your plan to provide periodic reviews on the performance of investments; adherence of the fund manager to the investment's published style or discipline, related internal and external investment fees; and material changes in the organization's investment philosophy and key personnel for each of the investment alternatives offered.
b. Describe your fiduciary responsibility to MPHA plan participants.
c. Describe how your firm defines and measures the success of retirement plan consulting relationships.
d. Describe your process, experience with, and relevant issues in conducting a Plan Administrator search.
1. In the past 36 months, how many Plan Administrator/Record Keeper searches have you conducted for plans your firm supports?
- How many resulted in a change in PA/Record Keeper?
- How many were executed within a government procurement framework?
- List the top three PA/Record Keepers/Trustees that you have recommended.
2. How many different Plan Administrators does your firm work with to support current clients?
3. What experience do you have with MPHA's current service provider, Wells Fargo?
Tab 6: Women-/Minority-Owned Business Enterprises and Section 3 Participation (Attachment C) and Equal Employment Opportunity Policy
Attachment C must be fully completed and submitted as part of the proposal submittal. The proposer must also submit a copy of its Equal Employment Opportunity Policy or Affirmative Action Plan.
Tab 7: Section 3 Preference (Attachment D) (Optional)
Section 3 is an income-based program through the U.S. Department of Housing and Urban Development that allows MPHA to give preferences to those that qualify. To claim a Section 3 Preference for this RFP, the proposer must complete and submit Attachment D along with any documentation required by the form.
Tab 8: Subcontractor/Joint Venture Information (Optional)
Identify whether your firm intends to use any subcontractors to perform these services and/or if the proposal is a joint venture with another firm. All preceding information required from the proposer must also be included for any major subcontractors (10% or more) or any joint ventures.
Tab 9: Other Information (Optional)
The proposer may include any other information that they believe is appropriate to assist MPHA in its evaluations.
Proposal Evaluation Factors
The following Factors will be utilized by the Agency to evaluate each proposal submitted.

Maximum Point Value = 30
Qualifications and experience of staff assigned, general organizational capacity, and approach to the services required.
No. 3: Past Performance, Industry Recognitions, and Client References
Maximum Point Value = 30
Past record of performance, industry recognitions, and client references.
No. 4: Section 3 Business Preference
Maximum Point Value = 15
Section 3 Business Preference (details in Section 8.1 of the RFP document).
