Bid Submittal Format
Bidders must submit one (1) copy of the bid submittal, including extending tabs, in a sealed package, properly addressed.

So that the Agency can properly evaluate the offers received, all such hard-copy bid submittals must be formatted in accordance with the sequence noted below. Each category must be separated by index dividers numbered (which number extends so that each tab can be located without opening the bid). None of the proposed services may conflict with any requirement the Agency has published herein or will issue by addendum.
Tab 1: Form of Bid.
This Form is attached hereto as Attachment A to this IFB document. This 4-page Form must be fully completed, executed where provided thereon and submitted under this tab as a part of the initial bid submittal.
Tab 2: form HUD-5369-A (11/92), Representations, Certifications, and Statements of Bidders, Public and Indian Housing Programs.
This Form is attached hereto as Attachment B to this IFB document. This 4-page Form must be fully completed, executed where provided thereon and submitted under this tab as a part of the bid submittal.
Tab 3: Bid Bond.
If necessary, the required Bid Bond as detailed within the following Section 3.4.1 herein.
