We hereby correct the following information published within the Attachments:
(1) Please remove Attachment D, Statement of Contractor’s Qualifications. This form does not need to be included in your proposal submission.
MPHA received the following requests for information and hereby responds to each:
(1) QUESTION. The contents of the numbered tabs in Table 3 of the Request for Proposals (3.1 Proposal Format) conflict with the tab numbers/content in Attachment A. Which of these documents should we follow when assembling our response?
AGENCY RESPONSE. Please see attached revised Attachment A, Form of Proposal. Please replace this form with the one that was included in the original attachments in the eProcurement Marketplace.
(2) QUESTION. What has been the historical distribution of work procured from the pool for each discipline? (e.g. % mechanical / electrical / structural / civil).
AGENCY RESPONSE. 90% mechanical and 10% electrical for this specific contract. Structural and civil would be incidental.
(3) QUESTION. Must our firm apply for a “Professional Firm Registration” from the Board of AELSLAGID?
AGENCY RESPONSE. No, but individuals do need to be registered.
(4) QUESTION. We are in progress of attaining Professional and Liability insurance, is it sufficient to state “In Progress” under #13 & #14 on Attachment C?
AGENCY RESPONSE. Yes, but the successful proposer(s) will need to have the insurance in place at the time of signing the contract.
